To celebrate the launch of the Eastern Health Alliance and demonstrate our vision of integrated care, we have decided to let stories do the talking.

The three stories in the following pages – told by a hip fracture patient, a diabetic patient and a stroke patient's caregiver – show how patients would move through the Eastern Health Alliance system of the near future, and how the seamless care impacts them and empowers them in their journey to recovery.

Some of the programmes mentioned in the stories exist, others are new, and yet others are being planned. They show the burgeoning possibilities that mark a 'healthcare revolution' in eastern Singapore.

We hope you find the stories - and the promise of the Eastern Health Alliance - compelling.
